Hospice, Palliative, & Home Health Care During the COVID-19 Pandemic
The coronavirus (COVID-19) outbreak presents a difficult and uncertain period for health care professionals, patients and families in communities around the country. As the leading provider and trusted resource for home health, palliative and end-of-life care, Sangre de Cristo Community Care takes seriously its commitment to provide up-to-date, easy-to-understand information on the virus, its impact, and our evolving organizational response to the situation.
We are taking every possible precaution to protect our patients, their caregivers and our clinicians from the spread of the virus:
• Every clinician must pass a daily health screening before being approved for patient visits.
• We are employing the use of tele-health technology when appropriate, including tele-health admissions for hospice-eligible patients 24/7/365.
• We are conducting risk assessments on every patient who is referred to our service or currently on our service.
• We require personal protective equipment when treating patients with confirmed positive COVID-19 tests and those under home monitoring for coronavirus.
• We require weekly COVID-19 testing for all clinicians treating patients with confirmed positive COVID-19 testing.
• We’ve provided updated training on infectious disease control to all our employees, including the proper use of protective equipment.
• We’ve established a multi-disciplinary task force to coordinate and lead our efforts related to COVID-19.
From 24/7/365 care coordination and spiritual/psychosocial support, to seamless care transitions and intensive bereavement care, Sangre de Cristo Community Cares continues to deliver the services that patients and families rely on, even when we are unable to be physically present. Our chaplains, social workers and family services coordinators continue to consult with families, helping them cope with anxiety and anticipatory grief while their loved one is isolated. Our bereavement services and community support groups provide assistance for grief that may be heightened by an inability to visit or difficulty finding closure.
