Home Care We Provide Dementia Care Program

Dementia Care

This program is available to Medicare beneficiaries throughout Southern Colorado. Individuals may self-refer or have their healthcare provider send a referral for evaluation to see if they qualify for this new supportive program.

Benefits of the program:

Dedicated Care Navigator
24/7 access to a nurse line for questions
Caregiver training & support
Assistance with connecting to community-based services
Respite care for those who qualify

To qualify for this program, an individual must have:

Traditional Medicare
Dementia Diagnosis
Not be a resident of a nursing home, or enrolled in a PACE program

Dementia Caregiver Support Groups

Sangre de Cristo Community Care is proud to offer FREE virtual Dementia Caregiver Support Groups for both unpaid and paid caregivers supporting someone living with dementia. Join us on Thursdays from 2–3 p.m. for education, practical tools, and a compassionate space to connect with others who understand the unique challenges of dementia care. Sessions are held exclusively online via Teams.

Dementia Care

Hospice Care

Anyone can refer a patient to hospice, and early referrals greatly improve quality of life by allowing symptoms to be anticipated, managed, and prevented before they become severe. When hospice is involved sooner, patients and families receive more comprehensive support and can often avoid unnecessary hospitalizations or emergency visits.

Home Health Care

Sangre de Cristo Community Care provides award-winning home health care to families in Trinidad, Walsenburg, Cañon City, Pueblo West, and La Junta, offering skilled nursing, CNA support, physical therapy, and certified wound care through a compassionate, comprehensive team.

Palliative Care

Palliative care enhances a person’s care by focusing on symptom management for people with serious illness.
