When hospice patients need a higher level of care than can be safely provided at home, the Joni Fair Hospice House—our 10-bed inpatient hospice facility—offers a peaceful, supportive environment where symptoms can be stabilized and comfort restored. As the only inpatient hospice unit serving Pueblo and the entire Southeast Colorado region, we provide essential acute hospice care close to home.
Our certified and highly skilled hospice care team specializes in the medical management of pain and complex symptoms, offering 24/7 nursing support when urgent or intensive care is needed. This short-term level of care allows many patients to regain stability so they can return home or transition to a long-term care setting with comfort and dignity.
At the Joni Fair Hospice House, our focus extends beyond the patient—we care deeply for families as well. We provide dedicated support, meals, accommodations, and a welcoming environment where loved ones are treated with compassion and respect. Thoughtfully designed patient rooms and soothing common areas create a homelike atmosphere for all who enter.
Our ultimate goal is to help patients remain as comfortable as possible and, whenever appropriate, return to the place they cherish most: home. Dementia Care
This program is available to Medicare beneficiaries throughout Southern Colorado, and individuals may self-refer or have their healthcare provider send a referral for an evaluation to determine eligibility, offering benefits such as a dedicated care navigator, 24/7 nurse line access, caregiver training and support, assistance connecting to community-based services, and respite care for those who qualify.
Hospice Care
Anyone can refer a patient to hospice, and early referrals greatly improve quality of life by allowing symptoms to be anticipated, managed, and prevented before they become severe. When hospice is involved sooner, patients and families receive more comprehensive support and can often avoid unnecessary hospitalizations or emergency visits.
Home Health Care
Sangre de Cristo Community Care provides award-winning home health care to families in Trinidad, Walsenburg, Cañon City, Pueblo West, and La Junta, offering skilled nursing, CNA support, physical therapy, and certified wound care through a compassionate, comprehensive team.
