Refer a Loved One

Referrals to hospice may be made by anyone—including physicians, discharge planners, home health agencies, social workers, nursing homes, family members, friends, clergy, or even the patients themselves. Anyone who recognizes that a person could benefit from comfort-focused, compassionate end-of-life care is encouraged to reach out.

Too often, hospice referrals are made only when a patient is near death or experiencing uncontrolled symptoms. While hospice can provide essential support at this stage, patients and families experience significantly greater comfort and quality of life when care begins earlier.

Early hospice referrals allow many symptoms—such as pain, anxiety, shortness of breath, and nausea—to be anticipated and prevented before they become severe. In many cases, symptoms that might otherwise lead to hospitalization or an emergency room visit can be safely and effectively managed by our hospice team right in the patient’s home.

By intervening sooner, hospice care greatly enhances the patient’s comfort and provides families with valuable guidance, emotional support, and peace of mind throughout the care journey.
