There’s no place like home when it comes to providing expert care for you or your loved ones. Not only is home the most comfortable place to promote, maintain and restore health, having care at home also can minimize the effects of illness or disability.
Serving a variety of patients across Southeastern Colorado, including pediatric patients and those with developmental disabilities, Sangre de Cristo Community Care is known for delivering exceptional high-quality compassionate care with a strong team of local health care professionals, including skilled nursing, physical therapy and occupational therapy, providing a seamless opportunity for high-quality hospice, palliative care and home health services to residents in Crowley, Fremont, Otero, Las Animas, Huerfano, Eastern Pueblo and Western Bent counties. Services provided by Sangre de Cristo Community Care may include any or all of the disciplines outlined below.
SKILLED NURSING (SN): A Registered Nurse (RN) will complete a physical assessment and identify your care needs. Nursing services may include, but are not limited to, teaching on disease process and medication management, performing wound care, administering injectable medications, and assisting you in developing skills for self-care.
PHYSICAL THERAPY (PT): A Physical Therapist (PT) will assess and treat motor function limitations. Services of the therapists may include, but are not limited to teaching exercise programs, transferring, and walking safely (with or without assistive devices), and utilizing treatments such as massage, heat or ultrasound. Our therapists will design home exercise programs to help you recover from your illness, injury or surgery.
OCCUPATIONAL THERAPY (OT): An Occupational Therapist (OT) will assess and treat impairments performing activities of daily living (ADLs) while focusing on your individual goals. Services may include, but are not limited to, instructing proper use of adaptive equipment, educating on energy conservation techniques and evaluating your home and as applicable, making recommendations for barrier-free modifications.
SPEECH AND LANGUAGE THERAPY (ST): A Speech Therapy/Speech Language Pathologist will evaluate and treat speech, communication, cognitive, and swallowing disorders through a patient specific care planning process.
Home Health Aide (HHA): A Certified Nurse Aide (CNA) will assist you with your personal care needs and/or assists with medical treatments under direction and supervision of a RN, PT or OT.
